This school year, in place of a daily health assessment, we are requesting parents/guardians to fill out this questionnaire. This will only have to be done once per child for the entire school year.

The expectation is that you consider these questions daily prior to sending your child(ren) to school. It is important for us to stay vigilant as we continue to work through this epidemic.

Please complete this form before sending your child(ren) to school on September 7, 2021.
