For 2021-2022 School Year:

Sports Physicals will not be offered at school this year. 


Physicals required within 12 months of the start of sports.

Physicals required within 12 months of the start of school for the following grades: Any New student 1, 3, 5, 7, 9, & 11.

Vaccines requirements must be met within 14 days of the start of school. School exclusion is required by law if immunizations requirements are not met. Please check with myself or your child's health care provider to see if your child is up to date. Religious exemptions are no longer allowed in NYS. 
COVID-19 Vaccines are currently being offered to anyone 12yo and older. we were able to offer 3 clinics here at school last year and hope to be able to resume doing additional clinics this fall. If your child has had it, please let me know so I can record it in their cumulative health record. This is helpful to know should we need to perform contact tracing for positive cases. Thank you in advance!

NYSDOH COVID-19 Protocol

A student should remain home if displaying any ONE of the following symptoms:

-Fever > or equal to 100 degrees Fahrenheit or Feeling feverish or with chills

-Cough

-Loss of taste or smell

-Fatigue/feeling of tiredness

-Sorethroat

-Shortness of breath or trouble breathing

-Nausea, vomiting or diarrhea

-Muscle Pain or body aches

-Headaches

-Nasal congestion/runny nose
******If your child has a chronic condition/diagnosis that could cause any of these symptoms, please provide documentation from their health care provider. At times this is helpful to avoid unnecessary testing/missed days of school. However, at times, if the symptoms appear to be infectious and not potentially from the chronic condition, testing will be still be required.*******

If your child has any ONE of the above symptoms, they need to be evaluated by their health care provider.  DO NOT send them to school.

Scenario 1: Your healthcare provider gives an alternative diagnosis of a known chronic condition with unchanged symptoms OR a confirmed acute illness (examples Laboratory confirmed influenza, strep throat) AND COVID-19 is not suspected then a note signed by their HCP explaining the alternate diagnosis is required. Note: A signed note documenting unconfirmed acute illnesses, such as viral URI (upper respiratory infection) or viral gastroenteritis, will not suffice. 

Scenario 2: Your healthcare provider recommends a COVID-19 test, they MUST stay out of school and in isolation until the test result is back. If Positive, Public Health will be contacting you. If negative, as long as the symptoms are improving AND they are fever free for at least 24hours without the use of fever reducing medication, they may return to school WITH a note from their HCP indicating the test was negative OR provide a copy of the negative test results. 

Scenario 3: (not recommended) A COVID-19 test was recommended but not done OR not evaluated by their HCP as recommended-Person MUST remain in isolation at home and is not able to go back to school until the health department has released them from isolation which will be at least 10days after the start of symptoms AND symptoms are improving AND they are fever free for at least 72hours without the use of fever reducing medication.

If Your child has a pending COVID-19 test, they need to remain home until it is negative and you are able to provide me with a note from their HCP indicating the test was negative OR provide a copy of the negative test results. Even if a Rapid COVID-19 test was performed and was negative, if they have a "send out" COVID test pending, they are to remain home.

Routine Screenings for Hearing, vision and scoliosis will be performed this year. You will receive a call with a follow up letter in the mail if any concerns are noted. Screenings are performed on new entrants, along with any students in grades 1, 3, 5, 7, 9, and 11 if their Primary Health Care provider did not do it at the time of their health exam. Screenings may also be requested by parents and/or teachers/staff of any student if there are concerns.
